Required Sport Participation Forms
Health forms must be up to date within a 12-month period. For example, if a child’s physical form is dated 9/15/22, then the physical form will expire 9/15/23. It is recommended that parents turn these forms in at the start of every school year (by July 31.) Sport Physical Forms must be up to date prior to tryouts.
These forms need to be uploaded to the DragonFly app/website, see information below on setting up an account.
This year our student athletes paperwork will be housed using DragonFly Max. DragonFly Max is an web browser and app that houses student athlete information. Please see the below form and follow the steps to create a DragonFly account for your student athlete. All parents must have an active account prior to try outs in order for your student athlete to participate. Please reach out to [email protected] if you have any questions about setting up your account.
